29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: (a) On or about April 16, 2017, and times prior thereto at Plant #3- Tunnel Kiln; employees were required to enter a permit required confined space exposing employees to hazardous conditions within the space. (b) On or about April 14, 2017, and times prior thereto at Plant #3- Tunnel and Roller Kiln Dryers; employees were required to enter a permit required confined space exposing employees to hazardous conditions within the space. (c) On or about April 14, 2017, and times prior thereto at Plant #3-Clay Silos and Bins; employees were required to enter a permit required confined space exposing employees to hazardous conditions within the space. (d) On or about April 14, 2017, and times prior thereto at Plant #3- Tunnel and Roller Auger Press Pits; employees were required to enter a permit required confined exposing employees to hazardous conditions within the space.
