29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems. Employees working at elevated heights in excess of 4 feet and more were not protected from falling. a) Ceiling Set: On or about 12 April 2017, an employee was performing work on a ceiling of a mobile home trailer under construction while wearing a harness but not connected to a lanyard or self-retracting lifeline (SRL). The height of the employee to the next lower landing was approximately 54 inches or 4 feet 6 inches. b) Ceiling Set: On or about 2 May 2017, an employee was performing work on a ceiling of a mobile home trailer under construction without the use of a harness or lanyard/SRL. The height of the employee to the next lower landing was approximately 54 inches or 4 feet 6 inches. c) Exteriors: On or about 2 May 2017, an employee was applying shingles to the roof of a mobile home under construction while standing on the top rail of the catwalk guardrail at the edge of the roof with a drop off to the concrete floor below at a height of approximately 127 inches or 10 feet 7 inches. d) Upper Mill: Employees access the mezzanine edge to place product in a wooden bin, place trash in a dumpster, and stack cut product for pickup, exposing the employee to a fall hazard of approximately 10 feet to the concrete below. e) Cabinets: Employees access the mezzanine edge to retrieve parts and place trash in a dumpster, exposing the employees to a fall hazard of approximately 14 feet to the concrete below. Cavco Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1), which was contained in OSHA inspection number 1043543, citation number 02, item number 001 and was affirmed as a final order on 09/23/2015, with respect to a workplace located at 2301 N. Highway 46, Seguin, TX 78155.
