29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) Live receiving: On or about 03/08/2017, and at times prior, the employer exposed employees to potential electrical shock hazards, in that a cover was not placed over the electrical outlet being used to power and industrial fan. (b) Outside chemical room: On or about 03/08/2017, and at times prior, the employer exposed employees to potential electrical shock hazards, in that the electrical outlet outside the chemical storage room was not completely covering the internal energized circuits.
