29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that are outside acceptable limits (defined by the process safety information in paragraph (d) of this section) before further use or in a safe and timely manner when necessary means are taken to assure safe operation. On or about February 12, 2017 and at times prior to, in the Sulfur Recovery Unit at Houston Refining in Houston, TX, the employer operated piping circuit 42-TG-1023-CAD (ISO439-0038-04), with a wall thickness less than required for safe operation. Houston Refining, LP was previously cited for a violation fo the Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.119(j)(5) which is contained in OSHA Inpsection Number 1139040, Citation Number 1 item 2, issued on September 15, 2016 with respect to a workplace located at 12000 Lawndale, Houston, TX. This file became final order on February 29, 2017.
