29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): On or about January 19, 2017, throughout the warehouse: Employees using a Toyota 8FGCU32 forklift to carry materials, molds, and other heavy equipment and supplies in the facility. The employer did not certify that employees completed a training course offered by the employer nor were their skills evaluated prior to operating the forklift.
