29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: The employer is failing to protect employees from struck-by and engulfment hazards associated with trenching and excavation. This was most recently documented on or about January, 20, 2017 at the jobsite located at or near Lot 27 on North Baltimore Ave. in Kansas City, Missouri. Excavations and protective systems are not inspected by a competent person prior to the employees entering the excavations. An employee working in a trench, which was approximately 8 to 13 feet in depth and approximately 43 feet in length was exposed to a collapse of the excavation walls.
