29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: Throughout the facility - On or about January 18, 2017, an evaluation of powered industrial truck operators were not conducted at least every three years, thereby exposing employees to struck by hazards. Dockside Steel Processing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(iii), which was contained in OSHA inspection number 891698, citation number 2, item number 1 and was affirmed as a final order on May 20, 2013, with respect to a workplace located at 11828 S. Stony Island, Chicago, IL
