29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: The employer is failing to protect employees from struck-by and crushed-by hazards associated with trenching. This was most recently documented on or about December 15, 2016 at the jobsite located at or near 507 Coleman Road in Belton, Missouri. The employer did not ensure excavations and protective systems were inspected by a competent person prior to the employees entering an excavation. An employee was working in a trench, which had not been inspected by a competent person, which was approximately 8 to 12 feet in depth, approximately 2 to 3 feet wide and approximately 20 feet long, sustained fatal injuries when the trench wall collapsed.
