29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated; the person which certified that the evaluation had been performed; the date(s) of the hazard assessment; and, which identified the document as a certification of hazard assessment. (a) Jobsite - On or about November 30, 2016, the employer did not have a written certification verifying that the required hazard assessment for PPE needed to protect employee's eye/face, head, hands and feet had been done.
