29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: Throughout the plant, industrial machines: on or about November 8, 2016, the employer did not inspect hazardous energy control (lockout) procedures periodically (at least once per year) to ensure they were adequate and being followed. Employees assigned to perform repairs and setup tasks (servicing) in the point-of-operation and associated danger zones of machines, including injection molders, were exposed to caught-in hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). The Suburban Plastics Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 975636, citation number 01, item number 003 and was affirmed as a final order on August 20, 2014, with respect to a workplace located at 1000 Enterprise Drive, Grenada, MS 38901.
