29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Jobsite - Employees conducting precast concrete operations from a supported scaffold were exposed to fall hazards, approximately 11 feet above ground level, without the use of a fall protection system. Noted on or about 10/18/16. The was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 994899, citation number 1, item number 2, and was affirmed as a final order on 4/1/15, with respect to a workplace located at 787 Swanson Street, Philadelphia, PA 19147.
