Text For Citation: 02 Item/Group: 002 Hazard:
29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: Mixing Room, Donaldson Torit Dust Collector. On or about October 21, 2016, three authorized employees performing cleaning tasks on and around the dust collector did not each affix a personal lockout device to a group lockout device before servicing the equipment. Employees were exposed to amputation and laceration hazards from the unexpected energization of the rotary air locks on the dust collector. No alternative method of protection was available, such as machine guarding. Work Area Protection Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(f)(3)(ii)(D), which was contained in OSHA inspection number 1135795, citation number 1-3d and was affirmed as a final order on September 20, 2016 with respect to a workplace located at 2500 Production Drive, St. Charles, IL 60174. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
