29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. Crown Textile Acquisition, LLC - Toledo, Ohio: On or about August 30, 2016, employees were exposed to rotating part hazards and amputation injury in that the G.A. Braun Washer #3 (model 400 NTDP-2; s/n NTDP 40090120), was not locked out prior to inspecting the area of the machine's motor and brake clutch. The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to servicing tasks  the employer did not: - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and deenergiziation of the machine or equipment had been accomplished. Crown Textile Acquisition, LLC dba NuCentury Textile Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(4)(i), which was contained in OSHA inspection number 190551, citation number 1, item number 3b and was affirmed as a final order on July 24, 2012, with respect to a workplace located at 1 Southard Ave., Toledo, Ohio 43604.
