29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: At 355 Paul Avenue in St. Louis, MO 63135 (Fabrication Areas) - On or about 12/06/2016, the employer failed to receive the manufacturer's prior written approval where modifications and additions, such as, but not limited to, fork extensions on a "Toyota" (Forklift 5) powered industrial truck, which affect capacity and safe operation of powered industrial trucks were performed.
