Text For Citation: 02 Item/Group: 002 Hazard:
29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Production- On August 17, 2016, the employer failed to ensure that employees who performed maintenance on Press #62 affixed energy isolating devices to each energy source. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records. Chicago Powdered Metal Products Co. was previously cited for a violation of this OSHA Safety and Health standard, 29 CFR 1910.147(d)(4)(i) of the Occupational Safety and Health Act of 1970, which was contained in OSHA Inspection Number 1047751, Citation number 1, Item number 2a and was affirmed as a final order on October 8, 2015, with respect to a workplace located at 9700 Waveland Ave, Schiller Park, Illinois 60176.
