29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup, or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On April 18, 2016, the employer did not establish a lockout/tagout program that included an energy control procedure for employees to follow, training and periodic inspections to ensure that employees performing maintenance or clearing jams were protected from injuries caused by the unexpected startup of equipment, such as the conveyor system. Veterans Distribution of Chicago, Inc., was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.147(c)(1) which was contained in OSHA inspection number 839183, citation number 1, item number 2 and was affirmed as a final order on April 8, 2013, with respect to the workplace located at 2200 Arthur Avenue., Elk Grove Village, Illinois. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
