29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: (a) A competent person did not inspect for visible defects the I-beam used to support a swing-stage scaffold system. The I-beam sections were not connected and had visible gaps between the joints. These gaps allowed three (3) I-beam rollers to roll off the I-beam causing the scaffold system to fall approximately 45 to 50 feet to the next lower level.
