29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about October 27, 2015, and at times prior thereto; a. Employees in Building B were exposed to struck-by and caught-by hazards when operating an OKUMA L-9 serial number 4404-1488L without a chuck guard. b. Employees in Building B were exposed to struck-by and caught-by hazards when operating a MONARCH series 91 serial number 50722 without a chuck guard. c. Employees in Building B were exposed to struck-by and caught-by hazards when operating a MONARCH 20CM serial number 38246 without a chuck guard. d. Employees in Building B were exposed to struck-by and caught-by hazards when operating a vertical lathe BULLARD CUT MASTER 42 without a chuck guard. Alfa Laval, Inc. was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 314245945 Citation Number 2, Item Number 1 and was affirmed as a final order on August 22, 2011, with a final abatement date of August 16, 2011, with respect to a workplace located at 300 Chestnut Street, Lykens, PA 17048.
