29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Press #4 Stretcher Department: This violation was observed on October 15, 2015 in the press 4 stretcher department, where employees were exposed to point of operation hazards while operating the Cometal stretcher machine press numbers 2, 3, 4, 5 and 6. THE EMPLOYER WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1910.212(a)(3)(ii), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316092501, CITATION NUMBER 1, ITEM NUMBER 14 AND WAS AFFIRMED AS A FINAL ORDER ON 06/07/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 1715 SANDY LAKE ROAD, CARROLLTON, TEXAS 75006.
