29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section: a) Repelletizer #2; On or about September 1, 2015 and at times prior, the employer exposed employees to hazardous energies (including electrical, pneumatic and hydraulic), caught-in and amputations hazards in that the machine specific lockout/tagout procedure did not adequately outline the procedural steps for placement, removal, transfer and verification of lockout or tagout devices to control all energy sources when employees perform service or maintenance on the on the Repelletizer #2 at the extrusion department. b) Thermoforming AL-1; On or about September 1, 2015 and at times prior, the employer exposed employees to hazardous energies (including electrical, pneumatic), caught-in and amputations hazards in that the machine specific lockout/tagout procedure did not adequately outline the procedural steps for placement, removal, transfer and verification of lockout or tagout devices to control all energy sources when employees perform service or maintenance on the Thermoforming AL-1#2 at the Thermoforming department. Genpak, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 280226 citation number 1 item number 1 and was affirmed as a final order on August 1, 2012, with respect to a workplace located at 26 Republic Plaza Middletown, NY 10940.
