29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: a. At the jobsite, a safe means of access was not provided for an employee who climbed from a 2nd floor wall opening up and onto a carpenters bracket scaffold that was approximately 3 feet higher than the wall opening and 2 feet out and away from the wall opening, exposing the employee to a 21 foot fall hazard, on or about 8/26/15. FLG Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 950375, citation number 1, item number 1b and was affirmed as a final order on 3/14/2014, with respect to a workplace located at Post SOHO Square, 720 S Howard Avenue, Tampa, FL 33607.
