Text For Citation: 02 Item/Group: 002 Hazard:

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a) Campsite - On or prior to August 22, 2015, safety related work practices, including but not limited to, de-energizing and locking out electrical equipment, using appropriate electrical protective equipment and using appropriate personal protective equipment were not employed to protect workers required to operate, test, troubleshoot, replace and maintain electrical equipment at the facility. b) Campsite - On or about August 25, 2015 and December 4, 2015, covers were not properly fixed and secured to the field boxes creating openings and exposing live parts, throughout the campsite in the 120 Volt, 30 amp breakers field boxes, including but not limited to, the Panel 300 A field box which exposed employees to electrical hazards.
