29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Jobsite: An employee operating a Genie Model S-85 Aerial Lift, Serial Number S8510-008258 did not have a lanyard attached to the boom or basket and was fatally injured after being ejected travelling approximately sixteen (16) feet from the operator basket after the machine fell over while the basket elevated approximately forty-five (45) feet.
