29 CFR 1904.7(b)(4): When an injury or illness involved restricted work or job transfer but did not involve death or days away from work, the employer did not correctly record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and/or an entry of the number of restricted or transferred days in the restricted workdays column: a) On July 17, 2015, an employee sustained a work-related injury that resulted in 2 days of restricted duty or job transfer. A check mark was not placed in the column of the OSHA 300 Log for Job Transfer or Restriction (I), on or about July 22, 2015. b) On July 18, 2015, an employee sustained a work-related injury that resulted in 5 days of restricted duty or job transfer. A check mark was not placed in the column of the OSHA 300 Log for Job Transfer or Restriction (I), on or about July 22, 2015. c) On July 20, 2015, an employee sustained a work-related injury that resulted in 3 days of restricted duty or job transfer. A check mark was not placed in the column of the OSHA 300 Log for Job Transfer or Restriction (I), on or about July 22, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
