29 CFR 1910.1026(d)(1): The employer did not determine, in accordance with either paragraph (d)(2) or (d)(3) of this section, the 8-hour TWA exposure for each employee exposed to chromium (VI): On or about October 17, 2011, the employer failed to perform initial air monitoring for employees who weld using metals that contain chromium which can expose employees to chromium (VI).
