29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: Forge Shop and Hut, Wyman Gordon, 244 Worcester Street, North Grafton, Massachusetts; Employees were exposed to live electric equipment operating at 50 volts or more due to insufficient guarding provided by damaged wall hung distribution panels. The lack of protection for electrical panels exposed employees to electric shock, arc blast, and electrical fire hazards. a. Panel at column A-1-19, 120 volt panel - struck by Powered Industrial Truck exposing conductors observed June 8, 2015 b. Panel 2H, 240 volt panel, in Hut struck by Powered Industrial Truck exposing conductors observed June 10, 2015 Wyman Gordon was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(g)(2)(i), which was contained in OSHA inspection number 912469, citation number 1, item number 7a and was affirmed as a final order on August 8, 2013, with respect to a workplace located at 244 Worcester Street, North Grafton, Massachusetts.
