29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous: (a) At the Pia Marine Hotel project, there was no grounding prong on the extension cord that powered a 110 VAC Ridgid cut off saw and Makita square sander. The tools were powered through an extension cord missing the prong. The cord was plugged into an interior outlet. Employee was exposed to the hazards of electrical shock.
