29 CFR 1910.178(a)(5): Where a powered industrial truck was equipped with front-end attachments other than factory installed attachments, the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) Broad Street Bldg., on or about 3/5/15: Cat, Hyster, Clark and Allis Chalmers forklifts using a manufactured set of five foot fork extension attachments were not marked to identify the attachment and show the approximate weight of the truck and maximum elevation with the load laterally centered. Pacemaker Steel & Piping Co., Inc. was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA Inspection Number 314347816, Citation 1, Item 3(b) and was affirmed as a final order on November 29, 2010, with respect to a workplace located at 2 North Floral Avenue, Binghamton, New York 13901. Abatement documentation must be submitted for this item.
