29 CFR 1910.132(d)(2): The employer must verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. (a) On 27 February 2015, at the facility located 4020 Winchester Ave, Kansas City, Mo., the employer had not completed a written certification that a workplace hazard assessment had been completed and verified. As a result, employees were exposed to serious injuries from fall hazards, hearing and eye injuries by not properly assessing the workplace for needed Personal Protective Equipment. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
