Text For Citation: 02 Item/Group: 002 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a. Multiple injuries from fall hazards while attempting to install beam 11B6 to column IC2 in that the employer did not implement the field modification of beam 11B6 prior to installaiton, to compensate for th 1.77 ft. off set of the existing pipe rack column connection point of the beam west end, as recommended by the Ashland Peteroleum Co. Project engineer on 6/23/93. One feasible means of abatement would be to implement the field modifications as recommended by the Ashland Petroleum Company Project
