Text For Citation: 02 Item/Group: 002 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) At Ceres Terminal on the versel "Lydia", hatch #2 did not have a sufficient opening out of the hold to prevent hoisted drafts of pipe from striking agains the hatch and causing load to fall. Employees were exposed to the hazards of being struck by fallen pipe. Methods of abatement are but not limited to: a) Remove more pontoon hatch covers to allow for a larger hatch opening and for adequate clearance to remove drafts of pipe out of the hold.
