29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s) -------------------------------------------- INSTANCE A,C,D,F DELETED -------------------------------------------- (a) 60 Steamboat Boulevard, Manchester Pennsylvania, PIT numbers 15, 49, 45 - Employees operated powered industrial trucks that had missing and worn smooth contact pads causing materials to slip. The employer did not remove powered industrial trucks from service when found to in need of repair, on or about November 6, 2014. (b) 60 Steamboat Boulevard, Manchester Pennsylvania, PIT number 15 - Employees operated a powered industrial truck that had a broken key switch with exposed electrical parts. Employees receive a shock when turning the key. The employer did not remove powered industrial truck from service when it was reported to be unsafe, on or about November 6, 2014. (c) 60 Steamboat Boulevard, Manchester Pennsylvania, PIT number 30 - Employees operated a powered industrial truck that had an inoperable side shift. The employer did not remove the powered industrial truck from service when it was found to be in need of repair, on or about November 6, 2014. (d) 60 Steamboat Boulevard, Manchester Pennsylvania, PIT number 41 - Employees operated a powered industrial truck that had rail seals used as bolts for the clamp attachment (platen). The employer did not remove the powered industrial truck from service when it was found to be in need of repair, on or about November 6, 2014. (e) 60 Steamboat Boulevard, Manchester Pennsylvania, PIT number 41 - Employees operated a powered industrial truck that had bent forks (platens). The employer did not remove the powered industrial truck from service when it was found to be in need of repair, on or about November 6, 2014. (f) 60 Steamboat Boulevard, Manchester Pennsylvania, PIT number 84 - Employees operated a powered industrial truck that had a cracked or missing outer casing parts. The employer did not remove the powered industrial truck from service when it was found to be in need of repair, on or about November 6, 2014. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records. The Americold Logistics LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315740647, citation number 1, item number 1 and was affirmed as a final order on May 14, 2012, with respect to a workplace located at 86 Jackson Concourse Pendergrass GA 30567.
