29 CFR 1904.32(b)(2): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) was incomplete. At the inspection site, for calendar year 2009, the Summary of Work-Related Injuries and Illnesses, OSHA Form 300A, lacked the totals from the OSHA 300 Log for entries G through M6. All of the entries or lines were blank while the Log had two recordable cases, and should have reflected on the Summary. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the OSHA 300A Summary is complete and accurate.
