29 CFR 1926.1408(b)(4)(ii): The employer did not designate a dedicated spotter who is in continuous contact with the operator meeting the requirements in paragraphs 1926.1408(b)(4)(ii)[A] through 1926.1408(b)(4)(ii)[D]: Location: 556 Main Street Falmouth Lyberty Green Jobsite On or about October, 30 2024, employees were exposed to hazards associated with electric shock when there was no dedicated spotter used to ensure no part of the load, load line, rigging, or equipment gets closer to the power distribution line that the minimum approach distance.
