29 CFR 1910.147(c)(4)(ii)(D): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: Location: 123 South 20th St. Irvington, NJ 07111 - Shredder Line / Dust Room a) Employee was exposed to crushing hazards while performing servicing actions on a Vertical Baler Press Machine 6-2068, Shredder Line - Baler Press Machine and Fiber Control Snow Maker machine without having the proper procedural steps in place addressing the requirements for testing equipment to verify effectiveness and applying safe Lock out Tag out application inside the energy control panel on or about 12/22/21.
