29 CFR 1926.652(g)(1)(iii): Employees shall be protected from the hazard of cave-ins when entering or exiting the areas protected by shields. On or about September 18, 2019, employees entering an approximate 10 foot deep trench located on the northeast corner of E. Edgewood Dr. and Providence Ave. Appleton, WI 54913 were exposed to cave-in hazards as the portable aluminum extension ladder was located outside of the trench shield. Kruczek Construction Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1070301, citation number one, item number one and was affirmed as a final order on August 28, 2015, with respect to a workplace located at Bluemound Dr. and W. Pine St. Appleton, WI 54914.
