29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): (a) Options Medical Center dba Options, 1700 S. 120th St., Lafayette, CO 80026: On or before January 25, 2018, the employer did not provide employees with a fit test to determine the respirator they were required to use fit adequately. Employees spraying pesticides and fertilizers were required to wear a full face respirator and were not provided with a medical evaluation prior to doing so. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
