29 CFR 1910.218(f)(2)(ii): Die(s) were being changed or maintenance was being performed on hydraulic forging press(es) without the ram(s) blocked out: a) On or about the week of March 1, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the employer did not require employees to be protected from crushing hazards and the hazardous energy caused by gravity by blocking the ram during setup and dies changes on the 750 ton HPM hydraulic press.
