29 CFR 1926.1400(f): The employer did not establish, effectively communicate to the relevant persons, or enforce, work rules to ensure compliance with those provisions of the Standard that direct an operator, crewmember, or other employee to take certain actions. On or about July 5, 2012, procedures or policies were not established to ensure that the specific provisions of Subpart CC were being met when engaged in activities such as, but not limited to multiple crane picks under 29 C.F.R. 1926.1432. Employees were thereby exposed to the hazards associated with crane and load failures.
