29 CFR 1904.29(b)(3): A recordable injury or illness was not entered on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. At the inspection site, for calendar years 2010 and 2011, the OSHA 301 Incident Report or equivalent form was not completed within 7 calendar days of the date of injury or illness for recordable incidents. a) For calendar year 2010, case# 56442 did not have the date of completion for the form filled in. b) For calendar year 2011, case# 1609751 did not have the date of completion for the form filled in. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the OSHA 301 Incident Report or equivalent form is completed within 7 calendar days of a reported and recordable injury or illness.
