South Hadley, MA: On or about September 6, 2024, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed, and the signature of the person who conducted the training for an employee who was performing work on a residential roof and exposed to fall hazards of approximately 11 feet 7 inches.
