29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) On or about July 19th, 2024, the established procedure for the application of energy control was not done in sequence as required by 29-CFR 1910.147(d)(1)-(6) when employees were adjusting the chain of the Barelt U1701 Gum Pouch Machine in Clean Room 13 / Line 4. B) On or about July 19th, 2024, the established procedure for the application of energy control was not done in sequence as required by 29-CFR 1910.147(d)(1)-(6) when employees were cleaning the cooling bars of the Barelt U1701 Gum Pouch Machine in Clean Room 13 / Line 4. US Packaging, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1658327, citation number 1, item number 2-c and was affirmed as a final order on 8-8-2023, with respect to a workplace located at 401 East Morrissey Drive, Elkhorn, WI 53121.
