29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about October 2, 2023, employees at Dreco Inc. within the facility located at 7887 Root Rd. North Ridgeville OH 44039, were exposed to crushing and struck-by injuries in that the #203 Van Dorn plastic injection molding machine (Model 300-RS - Serial # 300-RS-25-2126) was not deenergized prior to servicing tasks such as, but not limited to, mold and die changeovers. The employer did not ensure that energy control application steps were implemented to control hazardous electrical, hydraulic, pneumatic, energy sources of the machine's mold, die, platen and associated moving parts, prior to the servicing task - the employer did not: ? Ensure employees had the method or means to control hazardous energy. ? Physically operate energy isolating devices and affix lockout device(s) to control the energy source(s). ? Verify that isolation and de-energization of the machine or equipment had been accomplished. Dreco Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.147(c)(1), which was contained in OSHA inspection number 1571772, citation number one, item number one and was affirmed as a final order on May 23, 2022, with respect to a workplace located at 7887 Root Rd. North Ridgeville OH 44039.
