29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Worksite located at 2 Applegate Drive, Robbinsville Twp, NJ 08691: An employee cleaned the Taeheung Engineering, Model TMI-CJ17P01, Serial Number TH17CJP01 without affixing locks to the energy isolating devices. The employee's right middle finger was partially amputated by the operating part of the machine. Violation occurred on or about August 20, 2023 The CJ TMI Manufacturing America, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(4)(i), which was contained in OSHA inspection number 1568863, citation number 1, item number 003 and was affirmed as a final order on 7/5/2022, with respect to a workplace located at 2 Applegate Drive, Robbinsville Twp, NJ 08691. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
