29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch: (a) Worksite: On or about 9/26/2021, the employer did not ensure that the tongue guards for abrasive pedestal grinding wheels were in place and adjusted to within one- fourth inch. West Warwick Welding, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 1187975, citation number 1, item number 2-e, and was affirmed as a final order on 12-22-2016, with respect to a workplace located at 970 Main Street West Warwick, RI 02893.
