29 CFR 1910.1053(g)(2):Where respirator use was required by this section, the employer did not institute a respiratory protection program in accordance with 29 CFR 1910.134: On or about June 2, 2021, employees were required to wear half-face tight-fitting respiratory protection with P100 filters while operating the cut-off saw. Employees have not received respiratory protection training in accordance with the standard. A). The cut-off saw operator was exposed to respirable dust containing crystalline silica at 62.3769 ug/m3 which is approximately 1.2475 times the permissible exposure limit (PEL) of 50 ug/m3; exposure calculations included a zero increment for 7 minutes not sampled. At this time employees are being exposed to levels above the PEL. The Avalon Precision Casting Company, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1000(c), which was contained in OSHA inspection number 1157899, citation number 2, item number 1/a and was affirmed as a final order on 11/4/2016, with respect to a workplace located at 15583 Brookpark Rd., Brookpark, Ohio, 44142.
