29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following element; however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): Mail Processing Area: On or about December 31, 2019, an employee was preforming a maintenance task on the APPS Machine without de-energizing moving parts on the slip torque conveyor. The employee was exposed to entanglement and/or amputation hazards from spinning rollers. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d), which was contained in OSHA inspection number 1229207, citation number 1, item number 2b and was affirmed as a final order on 10/04/2017, with respect to a workplace located at 1025 West 20th St. Oshkosh, Wisconsin 54092
