29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator and at least annually thereafter: On or about April 25, 2019, an employee was required to wear an N95 filtering facepiece respirator when wire brushing brass nuts and the employer did not provide a fit test. GRAHAM WELDING, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.134(f)(2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1224764, CITATION NUMBER 1, ITEM NUMBER 1c AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 5, 2017, WITH RESPECT TO A WORKPLACE LOCATED AT 813 E NORTH ST, DECATUR, IL 62521.
