29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (a) Options Medical Center dba Options, 1700 S. 120th St., Lafayette, CO 80026: On or before January 25, 2018, the employer did not provide employees with a medical evaluation to determine their ability to use a respirator prior to requiring respirator use. Employees spraying pesticides and fertilizers were required to wear a full face respirator and were not provided with a medical evaluation prior to doing so. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
