29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation: (a) 2901 Montrose Avenue, Laureldale Pennsylvania, 2017 OSHA 300 Logs - On 3/20/ 17 and employee received an injury to the right shoulder. The employer did not complete the OSHA 300 log in the required detail required in that no description of the injury or illness was provided and the object responsible for the injury was not documented in column (F), on or about August 10, 2017. (b) 2901 Montrose Avenue, Laureldale Pennsylvania, 2017 OSHA 300 Logs - On 3/28/ 17 and employee received an injury to the right lower ribcage. The employer did not complete the OSHA 300 log in the required detail required in that no description of the injury or illness was provided and the object responsible for the injury was not documented on column (F), on or about August 10, 2017. (c) 2901 Montrose Avenue, Laureldale Pennsylvania, 2017 OSHA 300 Logs - On 6/1/ 17 and employee received lumbar sprain. The employer did not complete the OSHA 300 log in the required detail required in that the object responsible for the injury was not documented in column (F), on or about August 10, 2017. (d) 2901 Montrose Avenue, Laureldale Pennsylvania, 2017 OSHA 300 Logs - On 8/2/ 17 and employee received an amputation to three fingers. The employer did not complete the OSHA 300 log in the required detail required in that the object responsible for the injury was not documented in column (F), on or about August 10, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
